The Mansfield District Hospital Support @ Home Program provides a range of basic support services to help maintain your independence and capacity to remain living in your own home and local community. Support is also available to carers to help strengthen their capacity to maintain their caring role.
Services that can be provided include:
- Personal Care
- Domestic Assistance
- Home safety and maintenance
- Transport services provided by volunteers
- Meals on Wheels
- Flexible Respite
- Social Support Groups
Funding for these services is available through the Commonwealth Home Support Program (CHSP), and the Home and Community Care (HACC) Program for Younger People (PYP). A referral for these services can be made by contacting:
- My Aged Care on 1800 200 422 or visit myagedcare.gov.au if you are over 65 years of age, or 50 years if you are a First Nations Australian.
- Support@Home team on (03) 5775 8800 during business hours if you are under the age of 65 years, or 50 years if you are a First Nations Australia.
- Or drop in to the Buckland Centre where you will find our Support@Home team.

The Mansfield Visiting Nursing Service (VNS) works on the basis that one of the best places to get well and remain independent is in your own home and provides nursing care to all people regardless of age, income or diagnosis.
VNS nurses aim to improve the health status of their clients enabling them to enjoy the benefits of remaining at home.
Visiting nurses provide palliative care, Hospital in the Home and post-acute care.
The service also aims to support benefit to family members and carers with frail, sick or confused older persons and can link families to other community agencies.
