Our Key Focus Areas
Community Health Nursing
Our Community Health Nurses provide a wide range of primary and preventative health care programs to the Mansfield, Jamieson and Woods Point communities. They work with clients either individually or in group programs to better understand and manage their healthcare needs. Some of our key programs include:
- Chronic disease management
- Parkinsons support
- Parent Child Mother Goose Parenting Program
- Walking for older adults
- Carers’ support
- QUIT education
Our Community Health Nurse is in:
- Mansfield on Tuesday and Thursday;
- Woods Point on Wednesday; and
- Jamieson on Thursday.
Diabetes Education
Our Diabetes educator works with individual clients to prevent and manage complications, and provide on-going monitoring as well as providing education for the general community to increase awareness of diabetes. Assessment of diabetes clients and customised management guidelines are also provided for patients and carers.
Dialysis
Mansfield District Hospital provides haemodialysis to patients in the Mansfield and surrounding area in a bright, friendly relaxed environment. Our service is a satellite unit affiliated with the Royal Melbourne Hospital Kidney Care Services. Our unit has three chairs operating on Mondays, Wednesdays and Fridays. Holiday dialysis is also available by arrangement.
Dietetics
Our Dietician provides nutritional advice for a range of dietary issues related to health problems is provided and works closely with our Diabetes Educator where appropriate.
Health Promotion
Primary Care Centre staff work with individuals, groups and the general community to support and provide a range of health promotion initiatives in response to specific local needs alongside key national and state health priorities.
Key initiatives focus on:
- Chronic disease management;
- Healthy Eating;
- Breastfeeding Awareness and Promotion;
- Mental Health and Wellbeing;
- Prevention of family violence; and
- Well Ageing
Mansfield Restart
Mansfield Restart is our innovative community-led and locally delivered program that deals with addiction and substance abuse.
PROGRAM AIMS:
The project aims to reduce drug use and the effects of use and addiction on individuals, family, carers and the broader Mansfield community.
It provides a supported opportunity for those affected by addiction to once again take charge of their life
HOW WILL WE DO THIS:
- By working with individuals and families to develop a plan of care to address their health needs
- By bringing together a multi-disciplinary team, including nursing, doctors, allied health and social support services to assist and support individuals and families
- By building resilience of individuals and families to allow them to once again take charge of their life
- Through sharing knowledge to empower individuals and families
- By enhancing community awareness of the harms associated with the substance use

Physiotherapy
Our physiotherapists provide an assessment and treatment service of movement disorders and mobility problems for individual clients. Assistance and suitable aids and appliances to help with mobility, rehabilitation and independence are available.
The following Physiotherapy services are provided at MDH:
MDH Acute Ward – 5 days per week – Muscoskeletal, pain management, strength and balance
MDH Midwifery – 5 days per week – Women’s Health and six-week post-natal check-up
Buckland House – 5 days per week – Pain management, strength and balance
Bindaree Residential Aged Care – 5 days per week – Pain management, strength and balance
Primary Outpatients
key eligibility criteria – Pension or Healthcare cardholders.
Goal-directed physiotherapy intervention for rehabilitation or chronic disease management.
Outpatient Group Therapy
- Able and Stable. Balance and falls prevention for people with history of falls, falls risk or fear of falling limiting participation in daily activities.
- Orthopaedic Rehabilitation. incorporating the GLAD (Good Life with Arthritis) program.
- Cardiac and Pulmonary Rehabilitation.
Podiatry
The centre podiatrist provides services for prevention, diagnosis, treatment and rehabilitation of foot conditions. These include wound care, walking assessments, orthotics and advice on appropriate footwear.
Social Work
Our Social Worker provides support for people with health related issues who require assistance with social supports and social issues. This may include advocacy, accommodation, finance, adjustment/loss and relationship issues. The Social Worker is available on Wednesdays and Fridays to assist by phone and/or face-to-face contact providing assessment of needs with further management and/or referral to other services as appropriate. Ongoing or short term support is provided where required. Appointments may be made in person or by telephoning hospital reception staff on (03) 5775 8800.
Social Work services are provided free of charge to residents of Mansfield and surrounding communities.
Speech Pathology
Our Speech Pathologists provide assessment and treatment for children under school age with communication difficulties;
The service can assist children having difficulty understanding instructions using words and sentences or helping children with unclear speech such as stuttering.
The service is also integrated into some local schools.
